FILE OF LIFE

If you still do not have a FILE OF LIFE in place, please stop by the front desk and pick up a packet.

For those members who do not know about the FILE OF LIFE:  In the event that you need to call 911 for an emergency at home, the FILE OF LIFE would provide vital medical information the moment emergency personnel arrive at your house.

The FILE OF LIFE packet contains a medical history form which you fill out, return to the magnetic packet, and attach to your refrigerator door.  The FILE OF LIFE sticker enclosed in the packet is for you to place near your front door.

Please don’t put off participating in this potentially life-saving program.  It is easy to do and it is free.  You will have the security or peace of mind, that in the event of a medical emergency, you or your loved one will receive a faster response and appropriate treatment.
